The Old Man character appears several times in the PTD to either help the player, give an item, or offer a trade after using certain Pokémon in specific levels.

- Cerulean Gym 1
- Old Rod (Challenge Level)
- After completing the level, the Old Man will give the Player the Old Rod.
- Vermillion City
- Trade: The Old Man will appear and offer to trade a Farfetch'd for your Spearow. The Spearow must be used at least once in the level. This is also based on an actual trade in the original games, though those original ones see the trade with a little girl in Vermillion.
- Route 2
- Lavender Town
- An Old Man is seen in the crowd, while Giovanni is holding his speech. It is unknown if it is the same man, or simply a stock character.
- Rock Tunnel
- Once the giant Onix has been defeated, the Old Man will appear at the entrance to Rock Tunnel. He will present you with the criteria for the Shiny Voltorb Quest.
- Route 15
- After completing the level, the Old Man will give the Player the Super Rod. He only appears the first time that you win this level.
- Viridian City
- Once again, an Old Man character is present in the group of characters surrounding Giovanni during the opening cutscene.
